A knocked-out (avulsed) permanent tooth is a true dental emergency, and the clock starts immediately. The first 30 minutes are critical for successful reimplantation. Here is exactly what to do:
Step-by-step emergency protocol:
1. Find the tooth and pick it up by the crown (the white part). Never touch the root — the fragile periodontal ligament cells on the root surface are essential for reattachment.
2. Gently rinse with milk or saline if the tooth is dirty. Do not scrub, scrape, or use tap water (chlorine damages root cells).
3. Try to reinsert it into the socket and bite down gently on a clean cloth. If you cannot reinsert it, keep the tooth in a cup of cold milk, saline, or between your cheek and gum.

Do not let the tooth dry out. Time and moisture are the two most important factors. Baby teeth should not be reimplanted. If a dental implant is ultimately needed, we can discuss that option after the initial emergency is stabilized.
Healing time depends on the type and extent of the bone graft. Here is what I typically advise my patients:
Healing timelines by graft type:
• Socket preservation graft: 3-4 months. This is done at the time of tooth extraction and is the simplest type.
• Ridge augmentation (lateral or vertical): 4-6 months. This rebuilds bone that has already been lost and requires more healing time.
• Sinus lift (lateral approach): 6-9 months. The upper jaw takes longer because we are lifting the sinus membrane and the graft must mature under it.
• Block bone graft: 4-6 months. Used for more significant bone deficiencies.
During healing, the graft material gradually integrates with your natural bone through a process called remodeling. We verify healing with follow-up imaging before scheduling your implant surgery. While bone grafting adds time to the overall implant process, it is absolutely worth it — without adequate bone, implants can fail. We would rather take the time to build a solid foundation.
